COMMUNITY RESOURCE NAVIGATOR
Abilene, KS Summary
POSITION SUMMARY
Position Summary:
The Community Resource Navigator is a Community Health Worker (CHW) and member of the Memorial Health System care team who helps patients identify and overcome health-related social needs that may impact their health outcomes. This position builds relationships with patients and families, connects them with healthcare and community resources, provides education and support, coordinates referrals and care plans, and follows up to help address identified barriers. The role works across Memorial Health System and in community settings, with a focus on supporting patients participating in the Nourish & Navigation Community Health Worker + Accountable Food Is Medicine program.
POSITION QUALIFICATIONS
Minimum Education N/A Minimum Experience Demonstrates proficiency with computers and standard office technology. Required Registration/License/Certification Valid driver's license Ability to complete Kansas Community Health Worker Core Competency Training through a Kansas Community Health Worker Coalition-approved educational provider within 12 months of hire date Ability to complete required Accountable Food Is Medicine training within 12 months of hire date
