What You'll Own:
Be the face of Constella for the members on your panel. You are the first person they meet, the one they call when things get hard, and the one who keeps showing up when others have stopped. Locate, engage, and build trust with members who are often disengaged from care, meeting them wherever they are: at home, on the street, in shelters, in coffee shops, in parking lots Conduct regular home visits and field-based outreach, and respond in real time when a member is in crisis or at risk of decompensation Help members navigate the day-to-day realities of their lives: transportation to appointments, benefits enrollment, housing applications, food access, and connection to community resources Support medication adherence through in-person check-ins, appointment reminders, and accompaniment when that's what a member needs to actually make it through the door Serve as the bridge between members and the rest of the pod. Flag clinical concerns to the clinical team and Social Worker early, and warm-handoff members when the situation calls for a higher level of support. Document every visit, call, and interaction thoroughly and on time in our EHR so the rest of the team can pick up where you left off Contribute to how we build Constella. You'll be one of our first CHWs on the ground, and what you learn in the field will shape how we train and support every CHW who comes after you.
Who We Need:
Someone with real comfort working in the community and with people who are often mistrustful of the healthcare system, and for good reason A person who can build trust quickly and keep it over time, including on the days when a member is frustrated, in crisis, or hard to reach Someone with deep community ties, or a background in crisis response, substance use services, homeless outreach, peer support, or nonprofit work serving vulnerable populations A problem solver who doesn't wait to be told what to do. If a member needs a ride, a phone charger, or a sandwich before they'll talk, you figure it out. Someone who can hold steady in hard moments. You've seen acuity, you've seen trauma, and you know how to stay calm and kind through both. A team player who treats the NP, Social Worker, and MA as partners, and who gives and takes feedback easily Comfortable with technology. You'll use a phone, a laptop, and an EHR every day, and good documentation is part of good care.

Members on your panel are engaged, reachable, and showing up for the care they need Early warning signs get caught in the field and surfaced to the pod before they become ED visits or hospitalizations Members are connected to the housing, food, benefits, and community resources that make stabilization possible Trust with members, with community partners, and with the rest of your pod that compounds over time Field insights that make Constella's model sharper for every CHW who joins after you
Your Superpowers:
2+ years of experience in community health work, peer support, outreach, case management, crisis response, recovery services, or a comparable role serving people with behavioral health needs, substance use disorders, or significant social complexity Deep familiarity with the Kentucky communities we'll be serving, including the resources and the realities CHW certification, peer support certification, or willingness to obtain within a reasonable timeframe Excellent interpersonal skills. You can meet someone where they are without judgment, and you know when to push and when to listen. Valid driver's license, reliable transportation, and willingness to travel across the service area daily Comfortable with a phone, a laptop, and basic documentation tools Based in Kentucky, in or near the service area
