Have you ever been told you're a good listener, a problem solver, and are resourceful? At NBCH we provide high quality care to meet the needs of our diverse patients. We offer a variety of services to the community, including primary & urgent care, women's health, pediatrics, adult medicine, behavioral health and dental care. The Community Health Worker (CHW) is a position within our Adult Medicine Department. The Community Health Work works with community agencies and health center staff to connect patients with services beyond medical care. The CHW assists our clinical team and helps patients navigate through the care management process. The CHW helps patients access transportation services, complete documentation required by other agencies, and navigate other services the patient may need. The CHW will also serve as an interpreter for the patients and participate in community outreach events when needed.
Who We're Looking For:
An Empathetic Listener who gains ideas and feelings that identify needs. A Comprehensive/Critical Listener who observes covert signs to recognize social and psychosocial needs. Someone good under pressure who responds appropriately in emergencies and or intense situations. Someone resourceful who assists in initiating referrals and utilizing available resources within and outside the facility. Communicates Effectively in a culturally competent manner Aware and sensitive to unique needs of patients with variety of related health conditions. A patient champion who defends and preserves the rights of patients. Someone organized and has time management skills Someone tech savvy who will document in the electronic health record.
What You'll Need:
High School Graduate or GED Experience Must possess a minimum of 2 years' customer service experience. Prior CHW experience is preferred. CPR Required. Fluency in Speaking/Translating Spanish or Portuguese. Either hold a current CHW Certification or be willing to begin process of obtaining within 1st year of employment.
