Job Description
The Community Health Worker (CHW) - Quality I will assist in the overall goal of increasing patient connections to services for: preventative cancer screenings, chronic diseases, and mental health, medical, oral health services provided at Greater Portland Health. The CHW - Quality I should approach work with empathy and understanding and provide appropriate navigation to connect patients to cancer screenings. This Community Health Worker will need to have a valid Maine driver's license to drive the mobile medical unit and its team into the community. Level 3 English required. High school diploma or GED equivalent required Knowledge of Microsoft Word and Excel required Experience with electronic health records desired Valid Maine Driver's License (we will conduct a review of your driving history)
