In this role, you'll serve as a trusted bridge between patients, their families, and the healthcare system - helping people navigate care, access resources, and build the skills and support they need to manage their health. What You'll Do Conduct home visits, phone check-ins, and in-clinic outreach to support patients in managing chronic conditions and care plans Help patients schedule and keep medical appointments, and accompany them when needed Connect patients with community resources (housing, food assistance, transportation, insurance enrollment, etc.) Provide basic health education on topics like chronic disease management, medication adherence, and preventive care Build trusting relationships with patients, especially those facing language, cultural, or socioeconomic barriers to care Document patient interactions and outcomes in the electronic health record (EHR) Collaborate with physicians, nurses, social workers, and care coordinators as part of an integrated care team Advocate for patients' needs within the healthcare system Participate in outreach events and community health initiatives What You Bring High school diploma or equivalent required; Associate's degree or CHW certification a plus Lived experience in or strong connection to the community served Excellent interpersonal and communication skills; ability to build rapport with diverse populations Comfort with basic computer use and EHR documentation (training provided) Valid driver's license and reliable transportation (if home visits are required) Ability to maintain confidentiality and work with sensitive patient information 1+ years of community outreach, healthcare, or social services experience preferred (not required for the right candidate) What We Offer Competitive salary Health, dental, and vision insurance Paid time off and holidays Retirement plan with employer match Ongoing training and professional development A mission-driven team committed to health equity
