The program uses specially trained Community Health Workers with prior criminal-legal system involvement to coordinate services. The FIT Program is modeled on the Transitions Clinic Network and works with correctional systems, local jails, community organizations, health centers, and reentry partners to connect participants to medical homes, mental-health and substance-use treatment, medications, and other reentry supports. Position Summary The Community Health Worker serves within the FIT Program and connects recently released individuals living with chronic disease, mental illness, and/or substance use disorder to appropriate health care services while helping develop comprehensive reentry plans with local reentry partners. Essential Duties and Responsibilities Provide individualized and group support, empowerment, education, and case-management services to people released from incarceration who have chronic disease, substance use disorder, and/or mental illness. Recruit formerly incarcerated individuals for primary care, substance use disorder treatment, and mental health services. Conduct periodic assessments related to substance use, physical activity, dietary habits, tobacco use, and social and economic resources. Transport ambulatory patients between homes, clinics, hospitals, or other social agencies when appropriate and necessary. Advise individuals regarding health-care and community facilities, assist with utilization of services, and conduct follow-up as required. Educate individuals with chronic illness regarding evidence-based standards of care and self-management and help strengthen relationships with the health-care system. Document work through appropriate recordkeeping consistent with program policies and procedures. Make referrals for needed services and facilitate access to community resources. Conduct home visits when appropriate and necessary. Communicate with formerly incarcerated individuals and the FIT team regarding project needs. Participate in outreach and advocacy in the community, prisons, local jails, and with local reentry service providers. Participate in FIT-related community programs and outreach/engagement efforts to identify individuals who may benefit from the FIT Program. Participate regularly in FIT team meetings and required ongoing training for community health workers. Assist with programmatic data collection for quality improvement and other administrative duties as assigned. Required Knowledge, Skills, and Abilities Experience providing information, education, intervention, and/or referral services to culturally diverse populations. Knowledge of community resources, crisis intervention, counseling principles, violence-prevention techniques, and case-management practices. Skills in assessing client needs, preparing documentation and written reports, and communicating effectively with coworkers, supervisors, clients, and the public. Ability to operate office equipment and use Microsoft Office and the internet. Ability to work flexibly, take initiative, manage stress, and work effectively in diverse community and correctional environments. Excellent organization, follow-through, verbal communication, and written communication skills. Education and Experience High school diploma or GED required.
