JOB SUMMARY
Community Health Worker (CHW) is a standard position within the organization and individual job duties/responsibilities will depend on the program assignment. All CHW positions will include interaction with the public through group events or individual service provision. This particular position will work under the Safe & Thriving Families Manager to support pre and postnatal programs.
PRIMARY JOB DUTIES
Contribute positively as a staff member of the organization. Must be dependable and reliable to report to work as scheduled. Professional representation of Public Health Solutions to the public. Participate in all required trainings and cross-train in other public health programs to support department operations. Perform other public health duties as assigned during routine operations and emergency response. Scheduled hours may vary. Willing to be flexible to adjust hours, including possibly coming early, staying late, or occasional weekend hours as needed. Adheres to strict confidentiality regulations. Other duties as assigned. Provide culturally competent, professional service to the public and individual clients. Maintain knowledge of current prenatal, postnatal, and newborn healthcare practices. Provide education and resources related to healthy pregnancy, newborn care, parenting skills, breastfeeding, and accessing needed resources in the community. Make referrals to appropriate agencies when concerns fall outside the authority or expertise of the local health department. Maintain all standards set by PHS related to language interpretation and translation. Track and report all data related to service provision & client encounters accurately and on time. Maintain excellent working knowledge of PHS Electronic Health Records system(s). Work efficiently and professionally with healthcare providers. Develop and maintain positive working relationships with community organizations who serve the same clients as PHS. Support collaborative projects designed to improve maternal child health in our district. Participate in local coalitions, committees, and workgroups related to maternal child health. Participate throughout the district in assessment, collaboration, and outreach and education activities related to: Prenatal care & wellness Labor and Delivery Postnatal care & wellness Newborn care & wellness General health & wellness for pregnant/postnatal moms and newborns Breastfeeding Provide individual and group educational sessions on topics of importance to client population.
KNOWLEDGE, SKILLS, AND ABILITIES

REQUIREMENTS
Proficiency in written and verbal English and Spanish is required for this position. (Bilingual)
EDUCATION
A high school diploma or GED is required.
EXPERIENCE
Experience working with the public, particularly in a one-on-one service setting is desired. Specific experience in social services or healthcare is a plus but not required for most positions.
OTHER:
A valid driver's license and auto insurance coverage is required and must be able to safely operate a vehicle. While performing the duty of this job, the employee is frequently required to sit, use hands, talk, and hear. This position is 100% on-site and does not include remote or hybrid work. May occasionally lift and/or move up to 40 pounds.
COMPENSATION
Initial compensation will commensurate with education, experience, and qualifications for the job. Compensation may be adjusted based upon job performance.
