- The Cumberland Community Health Worker (CHW) serves as a bridge between healthcare providers and the community, promoting health education and access to services.
- Vice President, Healthcare/ Healthcare Government Relations Analyst Essential Functions
- Conduct community outreach and engagement activities for rural health grant Work collaboratively with the project manager and team to maximize productivity Collaborate with the whole project team, contributing to the entire project lifecycle Document encounters, referrals, and follow-up actions in the case management database Maintain confidentiality, comply with HIPAA and agency policies.
- 1. Education/Degree Required
- High School diploma required. Working towards an associate or bachelor's degree in a relevant field preferred.
- Certified Community Health Worker in NJ (preferred). 2. Years of Experience Required
- Entry Level Position 3. Other Requirements
- Valid driver's license and reliable transportation.
- Ability to travel regularly between multiple community locations.
- Ability to work a flexible schedule, including occasional evenings or weekends, if applicable.
- Preferred Qualifications
- Bilingual skills are preferred to effectively communicate with the diverse communities served.
- In compliance with the NJ Pay Transparency law, the hourly rate for this position is $25.
