Forward Leading IPA (FLIPA) is seeking a compassionate, community-minded Community Health Worker (CHW) to be embedded on-site at East Hill Medical Center (EHMC) . In this highly impactful role, you will serve as a trusted advocate and connector, helping Medicaid members navigate healthcare and social service systems while improving health outcomes across our community. This position is funded through New York State's innovative Social Care Network (SCN) initiative, a groundbreaking effort designed to reduce health disparities and advance health equity by connecting healthcare and community services. What You'll Do ✔ Build trusting relationships with patients and families ✔ Conduct outreach and engage individuals who may need support services ✔ Complete Health-Related Social Needs (HRSN) screenings ✔ Connect patients to community resources and healthcare services ✔ Coordinate referrals and follow up to ensure services are received ✔ Support care management activities related to social care needs ✔ Collaborate with providers, care managers, and healthcare teams ✔ Document interactions and outcomes in electronic systems ✔ Advocate for individuals and help remove barriers to care ✔ Contribute to a whole-person approach to improving health outcomes What Makes This Role Exciting Work directly with patients and make a measurable impact Be part of New York State's transformational Social Care Network initiative Collaborate with healthcare providers and community organizations Gain valuable experience in healthcare, social services, and care coordination Join a mission-driven nonprofit dedicated to health equity and community wellness Help shape the future of integrated healthcare in Upstate New York What We're Looking ForRequired Qualifications High School Diploma or GED 2-4 years of experience in human services, healthcare, community outreach, case management, social services, or a related field Strong interpersonal, communication, and relationship-building skills Ability to work independently while collaborating effectively with a team Basic computer proficiency, including Microsoft Office applications Valid New York State driver's license and reliable transportation Preferred Qualifications Associate's Degree in Human Services, Social Work, Public Health, or a related field Community Health Worker experience Experience working with Medicaid populations Familiarity with community resources and social service systems Equivalent education, lived experience, and professional experience will be considered.
