Job Description
Must be able to travel to various locations Nassau County, Suffolk County, Queens, Dutchess County, Brooklyn Engages individuals in the community and assists in navigating and accessing health care and community services, adopt healthy behaviors, and overcome barriers to health. Assists in the maintenance, planning and implementation of the department's programs and services. Organizes, coordinates, and implements programs and events to educate, develop and foster public interest, understanding and good will in the community.
Responsibilities:
Participates in community outreach and engagement activities. Conducts outreach in the community and "meeting people where they are" including apartment/housing complexes, community sites. Builds trust and relationships with individuals in their community. Identifies needs and refers community members to programs, services and community-based resources. Addresses community, civic, business and service organizations; represents the System in community activities and meetings. Willing to travel to multiple sites as needed. Identifies and formulates areas for program development, coordinates and implements programs that address health priorities and health disparities, with a focus on mental health. Utilizes standard data collection tools and interviewing skills to identify, observe, measure, record and report data related to community health needs and priorities. Plans and implements programming to address identified community needs including, but not limited to fairs, seminars, training programs, conferences, workshops, and public forums. Ensures programming and information provided is current and at the appropriate literacy level and language. Communicates the outreach plan and strategy for a particular community. Meets regularly with primary contact and Northwell team to discuss and provide updates on program deliverables. Completes relevant training courses and certifications to aid in community program implementation, such as Mental Health First Aid. Fosters an ongoing relationship with community and faith-based leaders representing a broad and diverse demographic. Other duties as required to be included. Performs related duties as required. Qualifications:
High School Diploma or GED, required. Associate degree, preferred. Valid Driver's License in New York State required and a reliable form of transportation. 1-3 years of relevant experience, required. Bilingual, preferred. (Spanish Haitian Creole)
