This role will be focusing on providing information in the following areas: Bronx, Upper Manhattan, and Brooklyn. The pay for this position is $25/hr. In this role, you will serve as a trusted health advocate, providing culturally sensitive education, outreach, and support to individuals and families. Your energy and compassion will help bridge gaps in healthcare access, improve health outcomes, and foster community resilience. This contracted position offers an exciting opportunity to make a meaningful difference while developing your skills in public health and community engagement. We are looking for two people who can cover: 1) Upper Manhattan - Bronx 2) Brooklyn Duties Conduct outreach initiatives to engage diverse populations, including those with disabilities and underserved communities Provide health education on topics such as chronic disease management, preventive care, Medicare, and public health policies Assist clients with navigating healthcare systems, including EMR/EHR documentation and program enrollment processes Collaborate with community organizations to develop and implement health programs tailored to community needs and facilitate access to social services Collect data for program evaluation, monitor client progress, and maintain accurate records Promote community engagement through public speaking events, workshops, and culturally appropriate communication strategies Skills Experience in healthcare and social services is recommended Experience working with individuals with disabilities and from culturally diverse backgrounds Excellent written and verbal communication skills for client interaction, report writing, and public speaking Proficiency in CRM systems for data collection and documentation Ability to develop community programs that address local health priorities and foster community participation Those bilangual in Spanish-English (Bronx and Upper Manhattan) and Kreyol-English (Brooklyn) are highly encouraged to reply Demonstrated ability to work independently while collaborating effectively within multidisciplinary teams Join us in creating healthier communities through proactive outreach, education, and compassionate support.
