Job Description

Community Health Worker (Bilingual Spanish) Staten Island, NY | Monday-Friday, 8:00 AM-5:00 PM | Direct Hire Make a Difference in Your Community! Join a mission-driven nonprofit dedicated to improving access to healthcare, housing, and food for underserved communities throughout Staten Island. This is an excellent opportunity for someone passionate about community outreach, public health, and helping others. Entry level candidates are encouraged to apply!
Compensation:
$50,000-$55,000/year Entry-level candidates welcome Annual salary increases Growth and advancement opportunities
What You'll Do:
Conduct community outreach and health screenings Support Medicaid members and individuals with asthma or chronic health conditions Complete health assessments and questionnaires Conduct home visits (approximately 8 per month; some virtual) Connect community members with resources and health-related services Assist with environmental health initiatives and supply coordination Present educational workshops and community programs Maintain accurate records across multiple systems
Requirements:
Fluent in Spanish and English (Required) Must live in Staten Island, NY or within a few mile radius. Passion for community health and helping underserved populations Ability to work independently and stay organized Comfortable with outreach, home visits, and public presentations Tech-savvy and able to learn multiple systems quickly Healthcare, social services, community outreach, or customer-facing experience preferred
