Community Health Worker (Bilingual English/Spanish)
Location:
New York City (Niagara NY)
Pay:
$25.00 per hour Job Summary We are seeking a compassionate, reliable, and bilingual Community Health Worker (CHW) to join our team. The ideal candidate will provide community-based support by visiting patients in their homes throughout New York City's five boroughs. This role is focused on helping patients access healthcare resources, providing education, coordinating services, and improving overall health outcomes.
Responsibilities:
Conduct home visits with patients throughout the Niagara area in New York City. Build trusting relationships with patients and their families. Provide education on health conditions, wellness, and available community resources. Assist patients in accessing medical, behavioral health, and social service resources. Coordinate care with healthcare providers and community organizations. Document all patient interactions accurately and in a timely manner. Maintain patient confidentiality and comply with HIPAA regulations. Travel daily to assigned patient locations.
Qualifications:
Must be fluent in both English and Spanish. Valid New York State driver's license with reliable transportation. Willing and able to travel throughout Yorktown Heights and surrounding areas High school diploma or GED required. Previous experience as a Community Health Worker, Case Manager, Patient Navigator, Social Services Worker, or in a related healthcare field preferred. Excellent communication and interpersonal skills. Ability to work independently and manage a flexible daily schedule. Basic computer and documentation skills.
