Job Description
We are seeking a compassionate and skilled CPST Provider to participate in the ongoing development and enhancement of the Center's system of care for the severely mentally disabled adults through the provision of independent living support services. Facilitate the movement of Center clients to the least restrictive and least intrusive service environment. The CPST Provider will provide services for clients residing in independent living situations. Assure that the client maintains community placement and receives and enhances independent living, activities of daily living and socialization skills and provide community outreach as appropriate.
Minimum Requirements:
High school diploma or general education degree (GED); one to three months related experience and/or training working with SMD population. Valid Ohio driver's license, use of personal car, CPR review and PPD testing are required.Benefits:
