Job Overview:
Under the supervision of CHW, registered nurse or social workerJob Requirements:
High School Degree with progress toward a Community Health Worker Certificate, OR an Associate's degree in a health related field Basic Life Support for Healthcare Providers (BLS) Knowledge of priority neighborhoods Good communication skills and interest in the health of economically disadvantaged familiesJob Responsibilities:
Collaborates with and maintains knowledge of other health and social service agencies to meet identified needs of clients; makes appropriate referrals when indicated. Acts as a liaison between client and other health service providers. Works independently in the community setting. Establishes relationships with community groups to recruit program participants. Actively participates in the planning and implementation of community program events. Conducts client interview to identify potential or actual barriers to personal health and well-being; reports findings to clinical supervisor for assistance in formulating action plan using program guidelines; maintains strict confidentiality and adheres to HIPAA regulations at all times. Completes documentation of client interactions in a timely manner per department policy and adheres to Program Guidelines at all times for formulation of action plan. Communicates to other individuals and agencies the unique knowledge, duties, and responsibilities of the emerging role of community health worker.Other Job-Related Information:
