ABOUT THE ROLE
We're seeking energetic Community Health Workers (10 in total) for our new partnership with Highmark Health , with experience in community engagement. This position is offered through a partnership between Highmark Health in Pittsburgh, PA and IMPaCT Care. While the role supports Highmark Health's community health initiatives, you will be directly employed by IMPaCT Care.

CHW. WHAT YOU'LL DO
Build trust through consistent presence and culturally aligned, empathetic communication Identify and address barriers to care: food insecurity, transportation, housing, and more Connect patients to care teams and community resources; follow up to ensure needs are met Document all client interactions, needs, assessments, and outcomes in the
EPIC EMR
system Complete SDOH (Social Determinants of Health) screenings and update care plans in a timely manner Participate in integrated care huddles and collaborate with clinical teams Use feedback and performance data as tools for growth, actively refining your approach based on what the metrics show about client outcomes
WHAT YOU BRING
Required High school diploma or equivalent 1-3 years in community-based outreach, health education, or related work Valid driver's license, must own a vehicle, have car insurance Ability to pass background check and pre-employment screening Flexibility to cover multiple locations across the area Preferred Bilingual Spanish preferred One year of CHW experience Experience with an EMR system such as Epic Familiarity with Medicaid, D-SNP, waiver programs, and recertification cycles Experience with SDOH screening tools (e.g., Hunger Vital Sign, USDA Household Food Security) or case management platforms Knowledge of HEDIS (Healthcare Effectiveness Data and Information Set) metrics or risk stratification concepts Strong ties to community organizations: faith institutions, shelters, food banks, or mutual aid networks
WHAT TO EXPECT
YOUR
DAY-TO-DAY
This is a field-based position. You will spend the majority of your time in the community — in clients' homes, in hospitals, in clinics, and at community organizations. You will report regularly to Highmark Health locations for team meetings, training, and care huddles.
A typical week might include:
Client home or community visits Care huddle with the clinical team at partner site Documentation and care plan updates Phone and text outreach to clients between visits
Schedule:
Monday through Friday, 9am-5pm.
