Responsibilities Engage with community members to assess their health needs and provide culturally sensitive support Educate individuals on topics such as nutrition, addiction counseling, Medicare, and public health policies Assist clients in navigating healthcare systems, including scheduling appointments and understanding medical terminology Conduct outreach activities to promote participation in public health programs and initiatives Collect data for program evaluation using EMR/EHR systems and maintain accurate records of interactions Apply motivational interviewing techniques to encourage behavior change and enhance client motivation Collaborate with healthcare providers, social workers, and community organizations to coordinate care and resources Requirements Prior experience or training in nursing, social work, teaching, or public health is highly desirable Knowledge of public health law policy and familiarity with Medicare and public health program planning Experience working with individuals with disabilities or from underserved communities is preferred Strong communication skills for public speaking, community outreach, and data collection purposes Ability to understand medical terminology and basic life support procedures Proficiency in EMR/EHR systems for documentation and reporting tasks Motivated self-starter with a passion for community service and health advocacy Join us in creating healthier communities through compassionate outreach, education, and support.
