LifeStream Services, Inc. Community Health Worker SUMMARY Responsible for providing health information in the community, assisting individuals in accessing community resources to meet social health needs and advocacy. This Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $206,927,896.80 with 100 percent funded by CMS/HHS. ESSENTIAL DUTIES AND RESPONSIBILITIES

Other duties may be assigned. Duties and responsibilities include the following. Provide Health Information to Community

• Delivers health education sessions, workshops and presentations to individuals and groups within the community. Including staffing area health fairs as needed
• Become a Dementia Friends Champion and offer Dementia Friends sessions in the community
• Evaluates the effectiveness of health education programs and initiatives by monitoring program activities and outcomes to track progress toward established goals
• Represent LifeStream Services professionally in community meetings, outreach activities, trainings, and partnership-development efforts. Direct Services to Individuals
• Conduct home, community-based, telephone and other approved visits to provide support, encouragement, education, goal setting and follow up
• Collect and document required health, social-needs, and assessment information within the CHW scope of practice and in accordance with program guidelines using approved, standardized assessment tools
• Connect individuals to health resources and services and provide support with navigating the healthcare system
• Be certified as a SHIP Counselor and provide assistance with calls and walk-ins
• Maintain confidentially and follow HIPAA guidelines
• Follow agency customer service standards Advocacy
• Advocate on behalf of individuals to ensure access to community services, entitlements and benefits
• Maintain relationships with community resources and have working knowledge of services available in the community
• Recognize changes, risks, or needs requiring additional attention and escalate concerns to the appropriate supervisor or care-team member.
• Assist with identifying participants in grant projects or special programs including GUIDE and ABC Community

QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE

High school diploma/GED with one (1) or two (2) years' experience and/or training in a related field; or an equivalent combination of education and experience. Associate's degree in human services or related field preferred. Must be proficient in public speaking. Experience working with older adults, individuals with disabilities, vulnerable populations, health care providers, Medicaid/Medicare is preferred.

MATHEMATICAL SKILLS

Ability to apply basic mathematics, algebra, and geometry.

REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving concrete variables in standardized situations. Must possess conflict resolution skills and ability to develop positive working relationships with a wide range of constituencies.

COMPUTER SKILLS

To do this job successfully, an individual must have working knowledge of Microsoft applications, and to be able to quickly learn software programs for various home and community based programs.

CERTIFICATES, LICENSES, REGISTRATIONS

Must possess a valid driver's license. Must be covered, maintain coverage and provide documentation of current automobile insurance meeting LifeStream's amount requirement. Must be able to travel throughout the LifeStream service area independently and beyond the service area on occasion. Must be able to be certified in SHIP counseling and as a Community Health Worker (Indiana) upon hire or the ability to obtain within 3 months of hire. Must complete trainings to remain in compliance with program requirements.

Pay:

$24.00 per hour Expected hours: 40.0 per week
