UPMC is seeking a mission-driven Community Health Worker to support patients and neighborhoods across the Pittsburgh region. In this hybrid role, you will conduct outreach, home and community visits, and virtual follow-up to connect individuals with medical care, insurance, and social services. You will provide health education, help patients navigate the healthcare system, and address social determinants of health. Working closely with clinicians and community partners, you will document outcomes, support quality improvement, and advance UPMC's community and population health goals.
