Monday-Thursday 9:00 AM-6:00 PM and Friday 8:30 AM-5:30 PM EST Job Summary We are seeking a Community Resource Coordinator to support members throughout Georgia by helping identify and address barriers that may affect their access to care and community services. This is a highly field-based position, with approximately 90% of the role involving travel and in-home member visits. You will meet with high-risk members in their homes, complete non-medical assessments, identify needs such as transportation, home safety, and other social determinants of health, and connect members with appropriate community resources. You will also work with local organizations and community partners, participate in community events, and help build connections that support members within the communities they serve. Responsibilities Conduct in-home visits with high-risk members and complete non-medical assessments to identify barriers to care and social needs Connect members with community resources, social services, organizations, and other appropriate support Build and maintain relationships with community partners and organizations Coordinate and participate in community events throughout the designated region and, when needed, other areas of the Georgia market Document member interactions, collect information accurately, and complete required reporting Manage scheduling, travel, follow-up, and daily activities while meeting monthly home-visit expectations Travel throughout Georgia, with daily travel varying by assignment; occasional community events in Savannah, Augusta, or Macon may also be required Individual and team performance is measured in part by successful home visits completed each month, with approximately 60% of scheduled home visits expected to be successful Qualifications High school diploma or equivalent 1-2 years of related experience in community health, community outreach, social services, care coordination, service coordination, case management, member services, or a related field Knowledge of community resources, local organizations, social determinants of health, and community health practices Strong verbal and written communication, organization, and time-management skills Ability to collect and document information accurately and work with digital record-keeping or reporting systems Cultural competency and the ability to communicate effectively with individuals from diverse backgrounds Strong problem-solving skills and the ability to work collaboratively with health professionals, social service providers, and community organizations Reliable transportation and willingness to travel throughout the state of Georgia Training And Travel The first 3-4 weeks of training will be completed virtually, followed by one week of in-person training in Atlanta, GA. Hotel accommodations will be covered and meals will be reimbursed during the Atlanta training. The position is remote but primarily field-based, with approximately 90% of the role involving travel. Daily travel may range from approximately 5 minutes to 2 hours depending on the assignment. Mileage is compensated, and overtime may be required based on assignment needs. Additional Information This position supports a health plan serving members throughout Georgia and involves working directly with members and community organizations to help connect people with resources that can address barriers to care. Candidates must reside in Albany, GA and have reliable transportation for extensive travel throughout Georgia.
