This Community Health Worker role supports Biologics through a dual focus: providing virtual support to Biologics patients & assisting patients + their caregivers in a non-clinical. The individual will assist with community resource support, provide health literacy session programs, coordinate & lead monthly peer support group(s), and ensure accurate data entry in the system of record. The CHW will operate within scope to navigate healthcare systems, social services, and community resources; facilitate access to direct services; and support the delivery of health education sessions and materials. The role contributes to measurable health improvements and programmatic cost savings. Bilingual English/Spanish is strongly preferred to support patient engagement across Biologics patient population. Position Qualifications & Requirements- Minimum 3 years of relevant experience; High School Diploma required. CHW certification or training preferred.
- Proficiency in Microsoft Office Suite (Outlook, Excel, Word, Teams, PowerPoint).
- Strong interpersonal and communication skills.
- Experience in community outreach and health advocacy with direct patient interaction.
- Ability to guide and support individuals in accessing health resources.
- Problem-solving and decision-making skills.
- Highly organized and proactive.
- Flexible availability for multiple shifts and occasional travel.
- Commitment to privacy, confidentiality, and ethical standards.
- Professional communication across all organizational levels.
- Adherence to company policies and accurate metric tracking.
- Demonstrate behaviors aligned with McKesson I²CARE & ILead principle Reporting & Oversight
- Program Leadership oversees CHW work, provides resources, and tracks outcomes.
- Monthly meetings with Program Leaders and Frontline Leaders to review progress.
- Biologics may assign tasks and provide guidance.
