JOB SUMMARY
: The "Building an Innovative, Comprehensive and Multi-level Medication Therapy Management and Non-Medical Health Related Needs targeted Intervention Model" (MH-Rx) study is seeking a highly passionate Community Health Worker (CHW
  • certified individual interested in delivering Non-Medical Health Related Intervention for pregnant women with chronic hypertension or with hypertensive disorders of pregnancy.
The primary objective of the MH Rx study is to conduct a randomized study evaluating the feasibility and impact of Non-Medical Health Related Needs targeted interventions and Medication Therapy Management on blood pressure control among pregnant women with chronic hypertension or with hypertensive disorders of pregnancy. The CHW offers health promotional interventions to individual patients and offers individual assistance in using mobile health technologies and blood pressure machines. Generally, the CHW provides direct support to the provides direct support to the principal investigators, leaders, and study team of MH Rx.
DISTINGUISHING CHARACTERISTICS
: The Community Health Worker of MH Rx study is a highly collaborative, self-motivated, creative person who is adaptable to new processes with capacity to function independently. This is a grant funded position. Essential Duties Summary Health promotions, Community Engagement, and Outreach (~70%) Cultivates and maintains relationships with participants and community partners. Provides Non-Medical Health Related Intervention for pregnant women with chronic hypertension or with hypertensive disorders of pregnancy. Prepare materials, set-up, plans, implement, and participate in outreach events and health promotional activities. Plans, implements and supports patient recruitment efforts Serves as direct contact for participants by phone, email, or website inquiry for all project programming. Communicates with the project team on participant requests, concerns, and updates. Administrative (~20%) Document encounters with participants. Collect sign-in sheets in health promotional events. Answers routine questions concerning program activities and operations. Screens and directs telephone calls and visitors to the appropriate person(s). Takes messages as necessary. Organizes and maintains the file system. Files correspondence, materials, and other records for future reference according to established file retention schedule and procedures. Assists with the coordination and tracking of pending assignments or requests for information from internal and external contacts. Verifies information or records and ensures accuracy and completeness. Report directly to the project PIs to support project activities. Works closely with the project PIs and other key personnel to prepare reports and deliverables to funding agency. Performs other job-related duties as assigned. Leadership (~10%) Provide training to students, interns, and volunteers on communication skills and health promotional activities. Provide/share talks to other CHWs on experience in the research projects at the national conference. % FTE 1.0 Hiring Range $45,486.69

Education Required:
Bachelor's degree in a health-related field (e.g., health, health promotion, community health, kinesiology, nutrition).
Required Licensing/Certification Required:
Texas Department of State Health Services Community Health Worker Certification or able to be certified within 2 months of hire
Knowledge, Skills, and Abilities Knowledge of:
Policies, procedures, and practices. Microsoft Office Professional or similar applications. Demonstrates an understanding of most work routines, rules, procedures, and operations. Maternal and child health population Chronic disease management (e.g., hypertension, dietary modification, physical activities) Skill in: Detail-oriented. Effective customer service. Problem-solving and decision-making. Multitasking and time management. Writing documents. Excellent verbal and written communication.
Ability to:
Analyze and interpret data. Prepare detailed reports. Work independently. Communicate effectively. Travel locally, regionally and nationally to community engagement and outreach events, programs, activities or conferences related to job function with cost reimbursable under grant funding Work Experience None Required.
