Education :
Required - High School diploma or equivalent. Preferred - 1 or more years of college-level coursework in a related field.
Experience :
Required - With a high school diploma or equivalent, 2 years of job experience.
Job Summary:
The Community Health Worker is responsible for providing outreach, education, and recruitment of eligible families for program services in Smith County while maintaining compliance with agency policies and procedures and established state/federal standards and requirements.
Essential Functions:
1. Determine eligibility of families for program services and state benefits. 2. Submit state benefit applications and supporting documents through YourTexasBenefits website. 3. Enter participant intake information into Salesforce and create a file. 4. Maintain a list of applications' status and updated the Salesforce database. 5. Provide safe, secure, and supervised transportation for the service population assigned to this position for transport. 6. Provide referrals to families based on needs identified while conducting outreach efforts. 7. Inspect company vehicles and fill out a checklist prior to using the vehicle. 8. Assist with coordinating and promoting Healthy Start sponsored events and meetings, including Consortium, support groups, and community events. 9. Build collaborative relationships with the community, health care providers, community leaders, service agencies, policymakers, funding sources, and target audiences. 10. Facilitate meetings and/or training for program families. 11. Communicate consistently and effectively with the program supervisor regarding any concerns or issues relating to community outreach efforts. 12. Maintain

Measurable Deliverables:
1. Document all daily outreach efforts, application assistance, intake, and consortium activities completely and accurately within two business days. 2. Enroll at least eleven prenatal clients monthly and six education clients monthly. 3. Conduct community outreach a minimum of four days a week. 4. Complete all required documentation and supporting documents for intake and state benefit application before completion of shift. 5.
Demonstrate the ability to:
a. Respond sensitively and competently to the service population's cultural and socio-economic characteristics. b. Communicate effectively in writing and verbally in English. c. Work in a fast-paced environment. d. Maintain computer literacy required to meet the responsibilities of the position. e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team. f. Travel as needed. 6. Other job duties as assigned per grant requirements.
Requirements :
1. Maintain certificates required for the position including but not limited to First Aid and CPR. 2. Valid Texas Driver's license with a clear driving record and reliable transportation. 3. Pass a pre-employment drug screen and random drug screens. 4. Provide proof or work eligibility status upon request. 5. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
