Resource Navigator Full-Time, Monday - Friday from 9am to 5pm Pay range: $17 - $19 dependent on experience Strength In Peers, a 501c3 peer-run, nonprofit organization, seeks a qualified individual to fill the Resource Navigator position within its Perinatal Health Hub. This position supports women who are pregnant, postpartum, or parenting infants by helping them access services and resources that promote maternal, infant, and family well-being. The Resource Navigator works out of the organization's Rocktown Health office in downtown Harrisonburg and includes community-based work and home visits throughout surrounding areas.
Position Summary:
The Resource Navigator provides direct community outreach, person-centered navigation, referral coordination, and follow-up support to help pregnant, postpartum, and parenting participants access healthcare, benefits, housing, transportation, and other resources, building trusting relationships and helping families move through complex systems with clarity and confidence.
General Job Duties:
Conduct outreach and engagement in community settings, partner agencies, and healthcare sites. Provide one-on-one resource navigation services to women who are pregnant, postpartum, or parenting infants and who may be experiencing barriers related to health, housing, transportation, benefits, childcare, or behavioral health, including facility-based, home-based, and virtual services. Complete screenings, intake activities, and participant needs assessments. Assist participants with applications, documentation, appointments, and follow-through on referrals. Coordinate and maintain communication with healthcare providers, case managers, and social service agencies, consistent with participant consent. Document participant contacts, referrals, and outcomes accurately and in a timely manner. Maintain professional boundaries, confidentiality, and compliance with agency policies and applicable regulations.
Minimum Qualifications:
1. High school diploma or equivalent required; associate or bachelor's degree in human services, social work, public health, or a related field preferred. 2. Experience in resource navigation, community outreach, case management, social services, or maternal health preferred. 3. Able to communicate effectively in writing and interpersonally in one-on-one and small group settings. 4. Strong computer skills, including data entry, word processing, email, and internet navigation. 5. Valid driver's license, reliable transportation, and acceptable driving record required. 6. Able to pass criminal background check.
