CHW will provide wellness coaching and holistic care to residents of the Triangle Neighborhood, a multi-racial, multi-cultural community of adults in subsidized housing. Working in a team setting, CHW is expected to work collaboratively with staff in assessing and meeting the daily needs of residents, building trusting relationships, and supporting the mission of the organization. For more details, visit trianglecommunityministry.org or contact director@trianglecommunityministry.org The Triangle Community Ministry (TCM) is a nonprofit service and advocacy organization serving the residents of the Triangle neighborhood located in downtown Madison, Wisconsin. The Triangle is the second largest subsidized housing complex in Wisconsin and is home to approximately 335 people. The population is a multi-racial, multi-cultural community of mostly older, single adults with high rates of disabilities. The mission of TCM is to serve as supportive companions to residents on their journey towards physical, emotional, and spiritual health. As a member of staff, the Community Health Worker (CHW) will be working in the TCM office and throughout the Triangle Neighborhood among residents of diverse backgrounds and abilities. The CHW will report to the Director and work closely with the Community Health Nurse and other community-based organizations in the Triangle Community to: Provide holistic care, wellness coaching and support to residents. Coordinate and support weekly Monday Meals (an established volunteer-led community meal program of TCM). Assist neighborhood residents in navigating the health and social services systems and increase self-efficacy while establishing trusting, supportive relationships. Advocate for individuals while also nurturing and encouraging self-advocacy to maintain independence, set and achieve goals. Provide culturally appropriate health information and education to residents, in collaboration with Community Health Nurse. Assist resident volunteers with weekly food distribution opportunities through the Community Development Authority (CDA - property management) and local charities.
