Phoenix, AZ Overview:

Our mission-driven healthcare organization in the Phoenix area is dedicated to improving health outcomes across diverse communities. In this Monday-Friday role, you will provide evidence-based Medical Nutrition Therapy in community and outpatient settings, conducting nutritional assessments, developing individualized care plans, and delivering culturally and linguistically appropriate counseling alongside a supportive interdisciplinary team.

Key Responsibilities:

Conduct comprehensive nutritional assessments, including medical history, dietary intake, anthropometrics, and relevant lab data Develop, implement, and monitor individualized Medical Nutrition Therapy care plans for patients with chronic and acute conditions Provide one-on-one and group nutrition counseling using motivational interviewing techniques Train, mentor, and evaluate local agency WIC staff, providing on-site and telephone technical assistance Oversee and approve food and formula special authorizations, coordinating with medical providers as needed Deliver culturally and linguistically appropriate nutrition education to diverse populations Document patient encounters, assessments, and outcomes accurately in the EHR Travel bi-monthly to clinic and community sites in the Phoenix area to provide on-site nutrition services

Qualifications:

Credentialed as a Registered Dietitian (RD) by the Commission on Dietetic Registration (CDR) Minimum 1 year of experience as an RD Valid driver's license and clean 5-year driving record required Must be able to pass a background check Knowledge of the WIC program Bilingual in Spanish preferred
