Job Description

Position Summary:
Provides outpatient nutrition or diabetes care and education services to patients at the Life Center, at our various clinics, at the inpatient unit, at the Residential Treatment Center or via telehealth. Organizes and teaches classes related to diabetes and weight management. Collaborates with and refers to other members of the health care team to promote optimal patient outcomes.
Critical Tasks:
Provides comprehensive and specialized medical nutrition therapy to patients with complex treatment and diagnostic needs. Provides hands-on instruction for all aspects of diabetes care and education, including intermittent and continuous glucose monitoring and the administration of insulin and other injectable diabetes medications. Teaches group diabetes education, weight management, cooking or other classes as assigned. Develops nutrition plans and implements interventions based on knowledge of patient's current health. Assists with development of objectives of care, education, standards of practice, and programs to assess outcomes of diagnosis and treatment plans. Utilizes health coaching skills, including motivational interviewing, to facilitate behavior change and improve patient outcomes. Completes nutrition assessment, identifies nutrition diagnosis, creates an intervention plan, and monitors patient progress and outcomes. Implements personalized, age-specific, and culturally appropriate nutrition strategies. Delivers client nutrition recommendations to physicians, nurses, and aides concerning patient treatment plans, established practices, policies, and procedures. Documents patient progress and shares with healthcare professionals. Works, as assigned, in our diabetes, pediatric, or other specialty clinics. Provides nutrition consultation, as assigned, to those in our inpatient unit or Residential Treatment Center. Collaborates, as assigned, with the Food Service Department in maintaining/updating inpatient or resident menus as needed, and acts as a resource in overseeing foods offered to patients on specialized diets. Maintains and updates patient's medical records through concise, accurate, and timely documentation. Works at other GRHC locations as needed. Performs other job-related activities as requested.
Required Qualifications:
Bachelor's degree in Dietetics, Nutrition or a closely related field of study required. Master's Degree in Nutrition/Dietetics preferred. Completion of an approved hospital dietetics internship program or equivalent required. 2 plus years' experience as Registered Dietitian Nutritionist preferred. Registered Dietitian Nutritionist, currently registered with the Commission on Dietetic Registration. Current Certification as a Diabetes Care and Education Specialist or ability to obtain within three years of hire. Certification in Health and Wellness Coaching preferred. Demonstrates ability to relate to diverse cultures and specifically the Gila River Community or other Native American cultures and community health services.
