Job Requirement Nutrition Assessment:
Evaluating patient diet intake, medical history, lifestyle to identify nutrition needs & potential deficiencies
Care Plan Development:
Creating personalized meal plans & diet recommendations based on individual needs, preferences, goals
Education & Counseling:
Providing guidance on healthy eating habits, food choices, impact of diet on health
Collaboration:
Working with physician, nurse, other healthcare professional to coordinate patient care
Monitoring & Evaluation:
Tracking patient progress, adjusting care plan as needed, providing ongoing support to achieve goals
Creating Education Material:
Utilizing resources such as handout, presentation, online content to promote healthy eating & address specific health concern
