Position Overview As the Registered Dietitian, you will be responsible for assessing participants' nutritional needs, developing individualized nutrition care plans, providing nutrition education, and collaborating closely with the interdisciplinary team to optimize participant health. This role includes both clinic-based responsibilities and occasional home visits to ensure participants receive comprehensive nutritional support. Responsibilities Complete comprehensive nutrition assessments and reassessments at least every six months or as participant needs change. Develop, implement, and monitor individualized nutrition Plans of Care (POC). Assess participants' access to food and coordinate services such as meal delivery, grocery assistance, and meal preparation resources. Recommend and order nutritional supplements when appropriate. Develop nutrition interventions for high-risk participants, including medical nutrition therapy recommendations. Provide nutrition counseling and education to participants, caregivers, and staff. Collaborate with the interdisciplinary team to support participant care planning and health outcomes. Monitor therapeutic diets, food intake, hydration status, and dietary modifications. Document nutrition assessments, interventions, and progress within the electronic medical record. Communicate diet changes and nutrition concerns with dietary staff, caregivers, and clinical team members. Participate in therapeutic dining initiatives and meal planning. Conduct kitchen sanitation and food safety evaluations. Review and monitor menus to ensure nutritional adequacy and regulatory compliance. Assist with dietary program administration, quality improvement initiatives, and regulatory requirements. Participate in care conferences, family meetings, wound care meetings, and departmental training. Qualifications Bachelor's degree in Nutrition, Dietetics, Food Science, or related field from an accredited college or university. Registered Dietitian (RD/RDN) credentials required. Minimum of one (1) year of experience working with frail or elderly populations. Knowledge of medical nutrition therapy and current nutrition practices. Experience completing nutritional assessments and documenting participant care. Strong communication, collaboration, and organizational skills. Ability to travel for occasional participant home visits as needed.
