What You'll Do Provide high-quality clinical nutrition care in a SNF/rehab setting Conduct assessments, care planning, and follow-ups Collaborate with interdisciplinary teams Maintain compliance and best-practice standards What We're Looking For Registered Dietitian (RD/RDN), active credential Confident, independent clinical decision-maker Team-oriented with excellent communication skills SNF experience preferred, but strong clinical skills are valued We are actively hiring and excited to connect with passionate RDs ready to join a supportive, growing team. Responsibilities include:
Working collaborative with the Certified Dietary Manager & Interdisciplinary Team Screening and nutrition assessment of all residents upon admission as well as quarterly and annual assessments. Developing Resident Centered Care plans Frequent monitoring of high-risk residents with enteral or parenteral nutrition or skin concerns. Participation with the IDT for weight variance & wound reviews. Providing oversight to the Dietary Manager for food service regulatory compliance and quality assurance. Knowledge & Education Requirements Knowledge of Nutrition Care Process, and standard charting methods for paper and electronic medical records preferred. Experience in LTC, clinical dietetics, and some food service background required. Knowledge of regulations for Food & Nutrition Services department such as Title 22, CMS, Dept of Health, etc. essential. RD/RDN credentials required as well as professional liability insurance.
