Job Description

Job Summary The Registered Dietitian (RD/RDN) is responsible for assessing, planning, implementing, and monitoring the nutritional care of residents in the skilled nursing facility. The Registered Dietitian collaborates with the interdisciplinary team to ensure residents receive appropriate nutrition services that promote optimal health, support medical treatment plans, and comply with all federal, state, and local regulations, including CMS requirements. Essential Duties and ResponsibilitiesClinical Nutrition Services Conduct comprehensive nutritional assessments for all residents upon admission and as required by facility policy and regulatory standards. Develop, implement, and monitor individualized nutrition care plans. Evaluate residents' nutritional status through assessment of dietary intake, laboratory values, weight trends, medical conditions, and clinical outcomes. Identify residents at nutritional risk and implement appropriate interventions. Monitor and document residents' progress and response to nutrition interventions. Provide recommendations regarding therapeutic diets, supplements, enteral nutrition, and hydration needs. Participate in care conferences and interdisciplinary team meetings. Consult with physicians, nursing staff, speech therapists, and other healthcare professionals regarding nutritional care. Regulatory Compliance Ensure compliance with CMS, state health department, and other regulatory requirements related to nutrition services. Maintain accurate and timely documentation in residents' medical records. Assist with survey preparation and participate in regulatory inspections as needed. Monitor compliance with physician diet orders and facility nutrition policies. Food Service Collaboration Work closely with the Dietary Manager and food service staff to ensure menu compliance and nutritional adequacy. Review and approve menus for nutritional content and regulatory compliance. Evaluate food preferences, cultural considerations, allergies, and special dietary needs of residents. Participate in quality assurance and performance improvement activities related to food and nutrition services. Conduct dining observations and make recommendations to improve resident satisfaction and nutritional outcomes. Education and Training Provide nutrition education and counseling to residents, families, and staff. Train dietary and nursing personnel on nutrition-related topics, including therapeutic diets, hydration, weight management, and food safety. Serve as a nutrition resource for facility staff. Quality Improvement Monitor facility nutrition indicators including significant weight changes, malnutrition risk, hydration status, pressure injuries, and supplement utilization. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Assist in developing and implementing policies and procedures related to nutrition services. QualificationsEducation Bachelor's degree in Dietetics, Nutrition, Food and Nutrition, or related field from an accredited institution. Completion of an accredited supervised practice program. Licensure/Certification Current Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR). Current state licensure/certification as required by state law. Current knowledge of CMS regulations and long-term care nutrition standards. Experience Minimum one (1) year of clinical nutrition experience preferred. Experience in skilled nursing, long-term care, rehabilitation, or geriatric nutrition preferred. Experience with electronic health records (EHR) systems preferred. Knowledge, Skills, and Abilities Strong understanding of medical nutrition therapy and geriatric nutrition. Knowledge of federal and state regulations governing long-term care facilities. Excellent assessment, documentation, and care planning skills. Ability to communicate effectively with residents, families, physicians, and interdisciplinary team members. Strong organizational and time management skills. Ability to work independently and manage multiple priorities. Physical Requirements Ability to sit, stand, walk, bend, and reach throughout the workday. Ability to travel throughout the facility to conduct resident assessments and dining observations. Ability to lift up to 25 pounds occasionally. Working Conditions Skilled nursing and rehabilitation facility environment. Exposure to healthcare and food service settings. Interaction with residents who may have cognitive, physical, or behavioral impairments
Pay:
$45.00 - $55.00 per hour
