Full Job Description Description Summary:
Plans, organizes, and conducts nutrition consults to home health patients based on patient need and physician orders. Functions as a member of the home care team with expertise in nutritional counseling.
Responsibilities:
1. Develops and implements a nutritional plan based on an assessment of nutritional needs of the patient, input from the patient and/or patient representative/caregiver, and current evidence-based practices. The plan is included in the patient's treatment plan and medical record and communicated to the physician and home health care team. 2.
Instructs home care staff to:
A) Provide appropriate counseling to patients/caregivers in the home setting; B) Evaluate and instruct on nutritional principles, dietary plans, and food selections; C) Adapt plans to the patient's lifestyle as appropriate; D) Re-evaluate nutritional care by providing home follow-up care. 3. Communicates effectively with all team members, patients/patient representatives/caregivers through conferences, individual consultation, and committee participation. This includes telephone, written, and electronic documentation. 4. Maintains confidentiality and safety of patient clinical records and information. 5. Attends case conferences and staff meetings as appropriate. 6. Complies with the organizations' policies and procedures. 7. Fulfills all compliance responsibilities related to the position. 8. Performs other duties as assigned.
Education Skills Experience:
Formal Education and Job-Related Experience:
� This position requires a minimum formal education of a master�s degree in nutrition or dietetics and minimum of six months� job-related experience.
License:
State of CT Registered Dietitian/Nutritionist license and current valid driver's license.
Required Skills:
� Proficient use of computer-based electronic medical records; ability to calculate figures and amounts, such as proportions, percentages, volume; ability to read, analyze, and interpret clinical documentation from members of the home health team.
Other Information:
Access to a good working vehicle to support visits to patients' homes.
