Responsibilities:
Assess and evaluate the nutritional needs of residents in a Skilled Nursing Facility (SNF) Create, implement, and monitor individualized nutrition care plans Provide counseling and education to residents, families, and staff Partner with nursing, therapy, and dietary staff to ensure coordinated care Document assessments, progress, and updates in medical records Ensure compliance with federal, state, and SNF dietary regulations Support food service operations to maintain safety, quality, and satisfaction
Qualifications:
Licensed Registered Dietitian (RD) in good standing Bachelor's degree in Nutrition, Dietetics, or related field Prior experience in long-term care or Skilled Nursing Facility (SNF) preferred Strong knowledge of clinical nutrition and therapeutic diets Excellent communication and teamwork skills Resident-centered approach to care How to
