Summary:
The Clinical Dietitian II works independently. This role is responsible for identification of patients at nutritional risk, planning and implementation of medical nutrition therapy for assigned population, determination of nutrition diagnosis, development and implementation of evidence-based nutrition care plans, monitoring and evaluating the patient's progress, and communicating with the interdisciplinary care team. Also, serves as a resource to Dietitians I.
Responsibilities:
Participates, independently or with minimal guidance, in performance improvement (PI) activities and quality initiatives, and demonstrates readiness to offer outcomes-based solutions to opportunities.

Develops and provides education, training, consulting, and counseling services throughout the organization and for the community. Develops presentations and serves as subject matter expert in internal and external education programs and symposiums.

Provides leadership in resolving nutrition care issues, establishing priorities, developing and evaluating programs. Serves as a resource to Dietitians I and reviews their documentation.

Provides training to Dietitians I, interns and students on nutrition care strategies and support food service staff competency. Serves as a preceptor, lecturer, or advisory board member for dietetic internship programs.

Collaborates with the Food Service department to provide nutritionally appropriate menus, food options, and services for patients.

Performs nutrition focused physical examinations and collaborates with the physicians on malnutrition diagnoses.

Participates in the development and revision of educational materials; provides recommendations to enhance learning activities and educational resources that elevate educational processes.

Collaborates with physicians, nursing, pharmacy and other healthcare team members and departments to determine, coordinate, and provide medical nutrition therapy. Communicates plans of care with patients and their families.

Assesses patients' nutritional status and needs. Develops and implements care plans, monitors patients' progress, and updates interventions. Completes thorough and timely documentation of patient sessions in the electronic health record.
