Job Overview We are seeking a passionate Registered Dietitian for our Maternal Infant Health Program to promote optimal nutritional health in our community. You will assess dietary needs, educate clients on healthy choices, and guide them in managing chronic conditions. Your expertise will directly empower healthy decision-making and support our families' journeys to wellness, transforming lives and fostering healthier communities. Responsibilities Conduct required monthly home visits to maternal clients and infants to provide support, resources, and healthy lifestyle education. Perform thorough nutritional assessments and educate clients on healthy eating, dietary modifications, and chronic disease prevention (including pre- and postpartum concerns). Collaborate with RNs and Social Workers to integrate, monitor, and adjust nutritional strategies within overall treatment plans. Maintain accurate, timely documentation of all interventions and client progress in electronic medical record (EMR) systems. Experience Proven clinical experience as a Registered Dietitian/Nutritionist, preferably within a Maternal Infant Health Program conducting home and telehealth visits. Experience managing patient service workflows and collaborating in multidisciplinary teams. Proficiency in EMR systems, healthcare documentation standards, and relevant medical terminology. Strong ability to communicate effectively and provide clear nutritional guidance to diverse clients.
