3007325751-2 Apply for this job now Job Description Christian Park Healthcare Center seeks a compassionate Registered Dietitian to support residents in our skilled nursing and rehab setting. The Dietitian will assess nutritional needs, develop and monitor individualized care plans, collaborate with nursing, therapy, and medical staff, and provide education to residents and families. This role supports quality outcomes, clinical excellence, and resident dignity by promoting optimal nutrition, specialized diets, and safe swallowing. Responsibilities Assess residents' nutritional status through clinical evaluations and chart reviews Develop, implement, and update individualized nutrition care plans Recommend and monitor therapeutic and texture-modified diets and supplements Collaborate with nursing, therapy, and medical staff on care planning and quality outcomes Document assessments, interventions, and progress per regulatory and facility standards Provide nutrition education to residents, families, and staff Participate in care conferences, QAPI, and regulatory surveys as needed Monitor weight trends, intake, and nutritional risk indicators Support safe swallowing plans in collaboration with Speech Therapy Ensure compliance with state, federal, and facility nutrition standards Required Skills Medical nutrition therapy Long-term care nutrition Nutritional assessment Care planning and documentation Therapeutic diets Texture-modified diets and dysphagia management Malnutrition identification and intervention Interdisciplinary team collaboration Regulatory compliance (CMS, state) Electronic medical record (EMR) documentation
