Pay Rate:
$40 / hour
Shift Details:
Hours will be worked between 8:00 am and 4:30 pm M-F. No weekend or holiday hours will be required. We are flexible on a 10-hour schedule if requested. Part-time or full-time hours are acceptable. Services to be Performed Provide medical nutritional therapy (MNT) for an assigned group of FSH patients. Utilize the Nutrition Care Process Terminology (NCPT) to provide nutritional care to assigned patients. Assess nutrition needs, integrate delivery of nutritional care to meet disease and age-specific requirements of patients within the assigned areas. Identify patients at high-risk based on criteria outlined by the facility. Provide Nutrition Education to patients related to the needs identified with NCPT in individual or group setting. Participate in patients' interdisciplinary treatment team review. Collaborate with members of the treatment team to ensure clinical nutrition services are met. Communicate with patients and team regarding MNT needs. Follow-up on nutrition screen for admissions and DMH transfers within 72 hours. Utilize electronic medical health record and paper medical record for documenting care provided. Establish and maintain effective professional working relationships with Dietetic Department employees and other facility employees. Communicate with food service regarding unique nutrition needs for patients. Share patient feedback on meals provided. Provide input on client complaints and grievances related to clinical nutritional services as needed. Serve as an active member on nutrition and health-related hospital committees. Maintain RDN and LD credential requirements including continuing education hours. Accepts other duties as assigned by Chief Clinical Dietitian or Director of Dietetic Services.
