Responsibilities:

Conduct patient assessments to identify nutrition needs and develop individualized meal plans Collaborate with healthcare team to implement nutrition plans and monitor patient progress Provide education and guidance to patients and their families on healthy eating habits and nutrition Stay up-to-date with latest research and developments in the field of nutrition and incorporate into practice Participate in quality improvement initiatives to ensure high-quality patient care

Qualifications:

Bachelor's degree in Dietetics, Nutrition, or a related field Experience working in a healthcare setting, with a focus on patient nutrition and education, preferably in a skilled nursing facility or similar environment, is preferred but not required.
