In this role, you will be responsible for providing comprehensive nutrition care, assessing resident nutritional needs, developing personalized nutrition plans, and ensuring compliance with long-term care regulations to enhance the quality of life for our residents. Key Responsibilities Conduct clinical nutrition assessments and develop individualized nutrition care plans for long-term care residents. Apply evidence-based Medical Nutrition Therapy (MNT) best practices to address complex clinical conditions. Complete Minimal Data Set (MDS 3.0) assessments accurately and in a timely manner. Document clinical notes, nutritional interventions, and care plans efficiently using electronic health record systems. Collaborate effectively with the interdisciplinary care team (physicians, nurses, speech therapists, and dietary staff) to optimize resident outcomes. Communicate with residents and their families regarding dietary needs, preferences, and nutritional education with exceptional customer service.

QualificationsEducation & Credentials:

Bachelor of Science (BS) or Master of Science (MS) in Nutrition and Dietetics. Successful completion of an ACEND-accredited Dietetic Internship program. RD credentialed through the Commission on Dietetic Registration (CDR) or RDE status (eligible to take the registration exam).

Skills & Clinical Knowledge:

Strong foundational knowledge of current best practices in Medical Nutrition Therapy (MNT). Excellent written and verbal communication skills. Strong organizational and time-management capabilities. Proficiency with computers and clinical software systems. High level of professionalism and customer service orientation.

Preferred Qualifications:
