Emerge Nursing and Rehabilitation located in Glen Cove, NY has an opportunity for a Registered Dietitian. This is a Full-time position

Requirements:
Certified Dietician/Nutritionist Responsibilities Include:
Evaluate the nutritional status of patients by reviewing pertinent information obtained from the electronic medical record, the hospital discharge papers, the interdisciplinary health care team, and family Apply knowledge of nutrition screening assessment to specific patient populations to define their nutritional needs Document pertinent findings in the medical record as per policy Provide quality nutrition counseling to patients, family members as indicated Implement and evaluate nutrition care plans for effectiveness; recommend changes as needed Use professional judgment to adapt as well as apply guidelines, protocols plus professional standards of care to the new changing needs of the patient Evaluate the need for enteral and parenteral nutrition support; providing recommendations as indicated Participate in interdisciplinary care plan meetings; communicating effectively with members of the health team Complete Minimum Data Set (MDS) documentation as requested by the facility Assist with discharge planning Menu planning as needed Knowledge of SIGMA Care preferred
Pay:
$85,000.00-90,000.00 per year
