Job Description
RESPONSIBILITIES
Responsible for all aspects of care of patients as part of the healthcare team. Nutritional care includes, but is not limited to, assessment, performance of nutrition-focused physical examination, nutrition care planning, education and its implementation and follow-up. Develop and implement nutrition care plan based on assessment of nutritional needs, current nutrition related problems and psychosocial status. Consult with health-care professionals concerning patient's nutritional care, evidence for malnutrition-related diagnoses, and recommendations for appropriate nutritional therapy. Counsel individuals and families in nutritional principals of their specific disease state, assist in meal planning, nutritional supplements and special product selection. Monitor pertinent laboratory values. Conduct objective studies (eq. Diet History) to assess nutritional status. Provide in-depth nutritional evaluation, inclusive of changes in nutrition-focused physical examination findings, and follow up continuity for assigned patients. Coordinate and implement nutritional care plans in accordance with Departmental Policies & Procedures and with all applicable regulatory agency codes to facilitate optimal nutritional care. Participates in patient care rounds & conferences. Communicate pertinent information to health care professionals regarding patient's nutritional status and management. Complete nutrition assessments, reassessments and physical focus exams as needed on a timely basis. Maintains and updates patient records in the Electronic Medical Record or charting system at each unit. Demonstrates timeliness, accuracy and neatness in maintaining records. Participates in ICU rounds and patient care meetings. Effectively communicates with nursing, medical and other healthcare team members to ensure achievement of patient care goals. Provides advice on nutrition related issues at committees/meetings. Works closely with Clinical Nutrition Team and Healthcare Team. Completes Nutrition Education Program for Healthcare Team in a professional and effective manner. Answers, returns calls and makes appointments with potential outpatient nutrition counseling patients. Completed Outpatient Nutrition Consult and Patient Education in a professional and effective manner. Documents visit and provides a written note in EMR and FAX note to referring physician. Establishes Nutrition Education Requirements and sets up follow up appointment with patient to encourage compliance to recommended number of visits. Completes Productivity Report documenting outpatient consults. Gather Data for Department Statistics. Completes Community Nutrition Education Programs as assigned in a professional and effective manner. Visits each offsite on a regularly established schedule. Works with personnel at each offsite in a professional, flexible manner scheduling appointments and follow-up visits as needed. Completed Documentation to EMR for each patient encounter. Completes admission, 90-day and annual progress clinical nutrition assessment in EMR on a timely basis (Dialysis). Reviews Monthly Lab Results and provides laboratory results report card to each patient on a monthly basis (Dialysis). Completed Departmental Statistics in a professional manner and on a timely basis. (Dialysis) Performs other related duties as assigned or requested.POSTION QUALIFICATIONS
Bachelor's Degree from a four-year college or university. Active Registration in the Academy of Nutrition and Dietetics, a professional organization, ensures standards with regards to the training and experience and continuing education. Renal Requirement - At least one year as a Clinical DietitianPHYSICAL DEMANDS

The starting salary range (or hiring range) for this position has been established based on relevant experience. The above salary range (or hiring range) represents SBSH's good faith and reasonable estimate of the range of possible compensation at the time of posting. Your total compensation goes beyond the number in your paycheck! Prior to start date, the selected candidate must meet the following requirements: Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Southampton Hospital's Employee Health Services- Complete electronic reference check with a minimum of three (3) professional references. Successfully complete a 4 panel drug screen
- Meet Regulatory Requirements for pre-employment screenings. Provide a copy of any required New York State license(s)/certificate(s). Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.
