  • Key Responsibilities:
  • Lead nutrition counseling sessions and develop individualized nutritional care plans.
  • Evaluate dietary needs and construct care strategies for patients.
  • Conduct screenings to assess nutritional risks.

  • Participate actively in Care Conferences to discuss patient nutritional requirements.
  • Maintain timely and thorough documentation for high-risk patients, including on weekends and holidays.
  • Ensure confidentiality and privacy at all times.
  • Engage in mandatory organizational training and meetings including safety and compliance.
