Job Summary:

We're looking for a passionate and personable Nutritionist to join our program. This role is all about taking a holistic approach to Wellness Care—focusing on the whole person, not just a diagnosis. You'll be working primarily with Medicare patients (65+), helping them build healthier habits, better understand nutrition, and make realistic lifestyle changes that actually stick. Key Responsibilities:

Create personalized nutrition plans based on each patient's needs, goals, and health conditions Provide ongoing coaching, encouragement, and accountability through the program Help patients understand nutrition in a simple, practical way they can apply daily Support patients managing chronic conditions like diabetes, heart disease, and weight concerns Track progress and adjust plans as needed Work closely with a care team to support overall patient wellness Keep clear and accurate documentation

Qualifications:

Degree in Nutrition, Dietetics, or a related field Registered Dietitian (RD) or similar credential preferred

Preferred:

nutrition-related certification or training in chronic disease prevention, health coaching, community health, diabetes education, weight management, or wellness programming. Strong communication skills—you're able to connect, motivate, and educate Someone who is compassionate, patient, and genuinely enjoys helping others
