In this vital role, you will provide expert nutritional guidance, develop personalized nutrition care plans, and support patients in achieving optimal health outcomes. Your expertise in nutrition, exercise guidance, and lifestyle interventions will help empower diverse communities to make informed dietary choices, improve overall well-being, and effectively manage medical conditions. This position offers an exciting opportunity to provide meaningful, patient-centered care while collaborating within an innovative healthcare setting focused on improving health outcomes through evidence-based practices. Responsibilities Conduct comprehensive nutritional assessments for patients across various age groups and cultural backgrounds. Develop and implement individualized nutrition care plans aligned with medical diagnoses, treatment goals, and patient needs. Provide education on healthy eating habits, dietary modifications, exercise, and lifestyle changes tailored to each patient's goals and health conditions. Deliver medical nutrition therapy and evidence-based dietary recommendations to support disease management and overall wellness. Collaborate with physicians, healthcare providers, and multidisciplinary teams to integrate nutrition care into overall patient treatment plans. Support continuity of care within a clinical environment that includes research-focused healthcare programs and initiatives. Utilize electronic health record (EHR) systems to accurately document patient assessments, progress, care plans, and outcomes. Provide nutrition counseling through in-person visits and telehealth platforms to ensure accessibility and patient engagement. Develop behavioral strategies to improve patient adherence and encourage sustainable lifestyle changes. Maintain current knowledge of medical nutrition therapy, medical terminology, telehealth practices, and relevant clinical guidelines. Communicate effectively with patients from diverse cultural backgrounds while providing respectful, culturally sensitive care. Skills and Qualifications Registered Dietitian (RD/RDN) credential required. Experience providing nutrition counseling, medical nutrition therapy, and individualized dietary guidance. Strong understanding of patient care principles and medical terminology relevant to nutrition therapy. Experience working with culturally diverse communities and delivering inclusive nutritional guidance. Knowledge of exercise, lifestyle interventions, and behavior change strategies to support long-term health improvements. Proficiency with electronic health records (EHR) systems and clinical documentation practices. Ability to develop comprehensive nutrition care plans tailored to patient needs and treatment goals. Excellent communication and interpersonal skills with the ability to build trust, motivate patients, and support positive health behaviors. Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification preferred or required based on organizational needs. Preferred Qualifications Experience working in a clinical, outpatient, specialty care, or healthcare research environment. Familiarity with telehealth nutrition counseling and virtual patient engagement. Experience supporting patients with chronic disease management, wound care, or complex medical conditions.
