Job Summary We are seeking a dedicated and knowledgeable Registered Dietitian to join our healthcare team. The ideal candidate will possess a strong background in clinical nutrition, chronic disease management, and medical nutrition therapy. As a Registered Dietitian, you will play a vital role in assessing patient needs, developing personalized care plans, and providing nutrition counseling to promote optimal health outcomes. Your expertise will support our mission to deliver high-quality nutritional care tailored to individual needs in the long term care settings. Duties Conduct comprehensive patient assessments to evaluate nutritional status and identify specific dietary needs. Develop and implement individualized nutrition care plans aligned with medical diagnoses and treatment goals. Provide evidence-based nutrition counseling and education to patients, families, and caregivers. Collaborate closely with healthcare professionals such as physicians, nurses, and therapists to coordinate patient care. Monitor patient progress and adjust nutrition interventions accordingly, utilizing diagnostic evaluation and nutritional analysis tools. Manage documentation within electronic health records (EHR) systems accurately and efficiently. Experience Prefer experience working as a Registered Dietitian with familiarity in long term care environments. Demonstrated expertise in medical nutrition therapy (MNT), especially for patients with chronic conditions such as diabetes, cardiovascular disease, or geriatrics. Experience with tube feeding management, dietary support for long-term care residents, and weight management programs. Strong knowledge of physiology related to digestion, metabolism, and nutritional requirements in senior age groups. Proficiency in electronic health records (EHR) systems and documentation standards essential for accurate patient tracking.
