Job Description
POSITION SUMMARY
The Registered Dietitian will provide medical nutrition therapy services and other educational and supportive programming for diverse populations with chronic and serious health conditions in a community-based setting. This position will work closely with the Executive Chef to establish and maintain standards, systems, and processes to ensure that the MTM program meets standards and requirements established by the Food is Medicine Coalition (FIMC) and aligns with California Food is Medicine Coalition efforts as applicable. The Registered Dietitian provides medical nutrition therapy services, assesses population needs, and brings an innovative mind-set to educational and supportive programming to promote client self-efficacy, behavior change and improve quality of life. This position is ideal for a self-motivated RDN with excellent communication, collaboration, and organizational skills who is compassionate about improving the health and quality of life of people with chronic and complex medical conditions and enjoys working with the culinary team. This position requires a level of comfort in communicating with clients on the phone and via telehealth. This opportunity is based at Teen Kitchen Project headquarters located at 8063 Aptos Street in Aptos, with the possibility of working some hours remotely. Essential Duties Supports the development of Teen Kitchen Project nutrition programs and services. Works collaboratively to develop systems for communication, coordination, and oversight. Work with clients to determine the most appropriate meal plan based on their medical conditions and other nutritional issues and needs. Provides medical nutrition therapy using the Nutrition Care Process and evidence-based practice, as applicable, to people with one or more complex or serious health conditions. Conducts interventions via telehealth, and/or telephone. Develops systems for ongoing monitoring and evaluation. Practice according to the Code of Ethics for Registered Dietitian Nutritionists and in accordance with all applicable laws and regulations. Responsible for establishing and adhering to documentation standards that meet professional standards (e.g., Standards of Practice and Professional Performance Standards for the Registered Dietitian) as well as all other requirements including, but not limited to, applicable laws and for health care claims. Implements select structured data (e.g., NCP Terminology) in nutrition documentation deemed important to informing client needs, services, evaluation, and impact. Collaborates with culinary team; inform menu plan development and authorizes that Teen Kitchen Project menus meet the nutritional needs of patients with a variety of health conditions including cancer, heart disease, congestive heart failure, diabetes, chronic kidney disease, and renal failure, as well as modifications to meet the needs of those with chewing and swallowing difficulties. Provides training materials and supports culinary staff and broader organization to ensure understanding of the how's and whys of executing medically tailored meals accurately; provides oversight on implementation of the menu plan. Gathers and assesses feedback from clients on the quality of the meals and works with the Executive Chef to approve new recipes to replace those that receive low ratings. Participates in MTM standards and oversight committees (internal and external). Develops and keeps current nutritional resources provided to clients. Maintains case notes and other required documentation for all client interactions, including nutritional assessments and medical nutrition therapy consultations. Participates in internal meetings as required and necessary. Qualifications A commitment to Teen Kitchen Project's Mission and Operating Principles Holds and maintains active Registered Dietitian/Registered Dietitian Nutritionist credential with the Commission on Dietetics Registration 2+ years of experience providing medical nutrition therapy for chronic medical conditions such as heart disease, kidney disease and diabetes Experience developing and delivering community-based nutrition and culinary education desired Excellent verbal and written communication. Able to communicate complex nutrition information simply and effectively Good organizational skills and attention to detail Very comfortable with technology Experience using electronic health records is preferred Proficient, or willingness to become proficient in the use of the Nutrition Care Process Terminology to document nutrition interventions, as applicable. with database systems for tracking client interactions Self-directed; comfortable working in a fast-paced collaborative environment Bilingual in Spanish strongly preferred; multiple language skills a plusESSENTIAL FUNCTIONS
