What You'll Do:
Conduct thorough nutrition assessments for patients at nutritional risk Develop and oversee personalized nutrition and hydration plans for residents Monitor changes in patient status, including weight, wounds, or new admissions Implement evidence-based nutrition interventions and document in medical records Evaluate the need for specialized nutrition supports and coordinate care with medical staff Participate in quality improvement projects and care conferences Contribute to survey readiness and uphold standards of care
What You Need:
Current Registered Dietitian (RD) certification Relevant experience in clinical, long-term care, or inpatient settings Strong understanding of JACHO, state, and federal nutrition regulations Excellent communication and teamwork skills Ability to adapt care to diverse patient needs, considering cultural and individual differences About the
