Responsibilities Conduct thorough patient assessments, including nutritional analysis and diagnostic evaluations, to develop tailored care plans that meet individual dietary needs. Provide nutrition counseling and health coaching to motivate patients toward healthier behaviors and improved self-management of chronic conditions. Collaborate closely with healthcare professionals such as physicians, nurses, and therapists to integrate nutrition support into overall treatment plans. Manage medical nutrition therapy (MNT), including specialized interventions like tube feeding management and dietary modifications for complex medical conditions. Document all patient interactions, assessments, and care plans accurately within electronic health records (EHR/EHR systems) to ensure seamless communication across the care team. Supervise and mentor junior staff or interns in dietetics practices, fostering professional growth within the dietary department. Stay current with the latest research in clinical nutrition, physiology, and dietetics to continually enhance patient care strategies. Skills Proven experience in clinical nutrition, including acute care, geriatrics, long-term care, or hospital settings. Strong knowledge of physiology related to digestion, metabolism, and disease processes affecting nutritional needs. Expertise in managing nutrition support procedures such as tube feeding and parenteral nutrition. Skilled in motivational interviewing techniques to effectively engage patients in their health journey. Excellent client communication skills with the ability to educate diverse populations about dietary modifications and lifestyle changes. Experience working with individuals with specific dietary needs or medical conditions requiring tailored nutritional interventions. Familiarity with EMR/EHR systems for documentation and patient record management. Supervisory experience or background mentoring dietetic interns or junior staff members. Knowledge of chronic disease management strategies including weight loss programs and disease-specific nutrition protocols. Ability to perform comprehensive patient assessments and develop actionable care plans that promote optimal health outcomes.
