Overview We are seeking a passionate and knowledgeable Registered Dietitian to join our dynamic team. In this vital role, you will deliver expert nutritional care, develop personalized care plans, and collaborate closely with healthcare professionals to promote optimal health outcomes for our residents. Your expertise will help enhance quality of life through tailored nutrition strategies and compassionate support. Responsibilities Conduct thorough patient assessments to evaluate nutritional needs and develop individualized care plans. Provide expert nutrition counseling and education to residents, families, and staff to promote healthy eating habits. Collaborate with healthcare teams to coordinate care plans for residents with chronic diseases, geriatrics, or specific dietary needs. Monitor patient progress using electronic health records (EHR) systems, documenting interventions and outcomes accurately. Establishes policies and procedures and provides administrative direction for menu formulation. Coordinates interdepartmental professional activities, and serves as consultant to management on matters pertaining to dietetics. Participate in audits, QAPI, and regulatory compliance activities. Qualifications Current registration as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN). Bachelor's Degree from an ACEND accredited program. Proven experience in clinical nutrition, including hospital or long-term care settings. Experience working with individuals requiring specialized diets such as tube feeding or other nutrition support modalities. Skilled in nutrition counseling, client communication, and motivational interviewing techniques. Familiarity with EMR/EHR systems for documentation and care coordination. Supervising experience is preferred, along with a background in geriatrics or acute care environments.
