Full-Time Salary:
$95,000 - $101,000 per year (based on experience) Join Our Team as an MDS Director (RN) Heartland Nursing and Rehabilitation Center is seeking an experienced and detail-oriented MDS Director (RN) to lead and oversee the MDS process while ensuring regulatory compliance and exceptional resident-centered care. This leadership role is essential in supporting accurate assessments, effective care planning, and strong interdisciplinary collaboration throughout the facility. Key Responsibilities Oversee the accuracy, completeness, and timely submission of all MDS assessments and documentation Ensure compliance with state and federal regulations, including Medicare and PDPM requirements Conduct comprehensive resident assessments and interviews Develop, implement, and monitor individualized care plans for both short-term and long-term residents Lead and participate in interdisciplinary care plan meetings Collaborate with nursing staff, physicians, specialists, caregivers, and families to optimize resident outcomes Provide leadership, education, and ongoing support to nursing staff Conduct in-service training related to MDS, RAPS, Triggers, PDPM, and Medicare guidelines Maintain MDS and care plan schedules for Medicare and non-Medicare residents Qualifications Active Florida RN license in good standing Minimum of 2 years of MDS Coordinator experience in a Skilled Nursing Facility Strong working knowledge of MDS, PDPM, Medicare guidelines, and care-planning processes Excellent organizational, leadership, and communication skills Benefits Generous Paid Time Off (PTO) Medical and Dental Insurance One (1) Personal Floating Holiday Employee Recognition Programs Supportive and team-oriented work environment Why Join Heartland Nursing and Rehabilitation Center?
